Cancellation Policy: You agree to pay a fee of $220 for non-attendance and/or cancellation less than 48 hours prior to appointment. Please note - in case of illness, appointment will be converted to Telehealth if you let us know as soon as possible.
Account payment:
**PAYMENT OF ACCOUNT IS STRICTLY DUE AT THE END OF THE CONSULTATION
You agree to pay the account in full on the day and at the end of the consultation. You agree to pay an additional accrued fee of $50 for each day, if for some reason the account is overdue. This will strictly apply if payment is not made at the end of the consultation.

Pre-reading of lengthy reports and completing any forms outside the appointment time will incur additional charges, which will be added to the consultation fee. Any such reports must be received at least 4 days prior to the consultation date.
We will set up your automatic Medicare rebate (only if you have uploaded the referral IN FULL), but we will not be responsible for rejected claims if details provided are incorrect in any way, so for your own benefit, PLEASE check that ALL Medicare PATIENT AND CLAIMANT details entered in the form are accurate and up to date (including CURRENT MEDICARE NUMBER/REF on card and DATE OF BIRTH of PATIENT AND CLAIMANT as well as MOBILE NUMBER TO WHICH PAYMENT LINK WIL BE SENT, PROVIDER NUMBER OF REFERRING DR). ***Please double check all digits in the numbers provided, as we find mistakes are common, and you will miss out on your rebate***
REPEAT PRESCRIPTIONS WILL NO LONGER BE POSTED OUT. YOU MUST BOOK A FACE-TO-FACE REVIEW APPOINTMENT IN ANTICIPATION OF THE TIME A NEW SCRIPT WILL BE REQUIRED (6 MONTHS IN MOST CIRCUMSTANCES).
TELEHEALTH consultations (when applicable) - you acknowledge and understand the limitations of this mode of consultation such as lack of physical examination. You consent to the consult proceeding by Zoom, understanding that a face-to-face consultation will be necessary subsequently.

SEPARATED FAMILY POLICY:
The assumption is that of shared parental responsibility regarding decisions affecting the child long-term unless you provide court documentation to the contrary.
For all medical appointments: It is recommended that both parents attend where possible.
Decisions regarding a child’s medical treatment (e.g. whether to commence medication or start intervention/therapy) should be made jointly wherever possible.
It is the attending parent's obligation to provide a copy of any relevant legal documents, court orders or other reports relating to the child’s management. If no such documents are provided, we assume shared responsibility.
Payment is to be made on the day of the child’s appointment -If there is legal agreement that requires the other parent to pay all or part of the treatment costs, it is the attending parent’s responsibility to settle the account in full and collect reimbursement from the other parent.
Bookings will not be made where the purpose of the assessment is to generate information for legal decision making around custody or parenting issues. No recommendations or reports generated as a result of our consultation can be used in any court or legal proceedings.
Either parent or legal guardian can: schedule an appointment with their child, be present at their appointment, and/or be sent a copy of relevant reports unless you present us with a court order restricting their involvement in the child’s care.
If the appointment is made by one parent, it is the strict responsibility of this parent to inform the other, invite them to attend, and provide feedback to them.
Privacy and information disclosure: Both parents are entitled to reports and appointment information on their child unless you provide us with a court order explicitly stating one parent is not entitled to medical information/correspondence.
It is expected that parents/carers will work together respectfully and cooperatively in the care of their child. We reserve the right to cancel any appointment or course of treatment where parental conflict is impacting on the care of the child, the ability of the clinician to treat the child in accordance with his recommendations or admin staff to carry out their job efficiently and without harassment or intimidation.

Privacy Policy:
The consultation will be audio-recorded using approved, secure cloud-based software to ensure precise documentation of all critical clinical details, enabling full engagement without the distraction of typing. The recording will solely enhance clinical notes and the written feedback/report for the referring physician. The audio will be handled strictly following privacy regulations and will be deleted, typically within 24-48 hours after the consultation. Should you have any objections, please voice them to us before the consultation begins and refrain from booking an appointment.
Personal information is used to assist in patient management as well as accurate identification and communication. The information may be disclosed to other professionals involved in the patient's health care, including general practitioners, specialists, allied health practitioners and providers of medical investigations. Information will be used for accounting and administrative purposes and to comply with Medicare requirements. You may visit and/or use the Services only if you fully agree to this Privacy Policy - and by accessing and/or using any of the Services, you signify and affirm your informed consent to this Privacy Policy, including to the collection and processing of your personal Information as explained.
Appointments are conditional on agreement to the terms above and you will be asked to do so when booking.
